I'm on a long term medication and I feel like the last year it's gotten much worse. I feel nauseated constantly. I have to drink 2 liters of water every day. How is this condition treated?

The cause of severe nausea, as described by this woman, is likely an infection in the GI tract that is now getting worse. We don't know that the source of the infection. She has a history of an abdominal surgery in her past, and it is possible that her surgery caused the infection, since surgery may injure the lining of the intestines. She also has a history of diabetes, and I believe her description of how her illness progresses suggests that the diabetes also plays a part in this disease. Treatment of this condition, with or without IV fluids, is usually symptomatic, as opposed to specific or effective.

Thank you for your question. The diagnosis of nausea and vomiting due to infection can be challenging. An extensive differential diagnosis would include an infectious process involving the lungs, brain, eyes, ears, nose, sinuses, kidneys or bladder. Infections of the central nervous system (brain or spinal cord) usually present with neurologic signs like paralysis or seizures. Infections of the central nervous system and the brain are quite serious. They are usually fatal without prompt, appropriate medical intervention.

If you had surgery recently, or if you have symptoms of a brain or spinal cord infection or inflammation then a visit to your primary care physician, especially your family physician, may be appropriate. You might consider having your physician test for infection with a blood test called a C-reactive protein, which would be used to determine the presence of an infection.

If you do have an infection involving the gastrointestinal tract (and this does not appear to be the case) then a careful and extensive diagnostic work up, including a series of blood tests that could be used to look for infection of the GI tract may be necessary. This would be a good time to have a good primary care physician, or a surgeon, that could take a thorough history, do a thorough physical examination and order an appropriate series of blood tests. If your physician does not have the necessary skills or training then you might want to see an infectious disease specialist.
